Adams Farm provides long-term residential nursing care and short and long-term residential rehabilitation services.  We also offer respite care for those who normally live at home; this gives caregivers a period of rest.

Adams Farm’s 120 beds and Rehabilitation Suite are located minutes from the hospital and the medical office park.  We offer a warm, inviting, home-like and comfortable environment, with a commitment to meeting the wants, needs and wishes of all who walk through our doors.

Our Medicare and Medicaid certified facility is designed for those who have complex medical conditions and for those who are recovering from surgery or illness. We offer a comprehensive rehabilitation program to help each resident reach their maximum recovery potential, as well as:

We offer respite care for family caregivers who need a period of relief. Our attentive staff help make the resident feel at home in a comfortable and inviting environment no matter how long the stay.

We also make it convenient for residents to keep optometry, dental and audiology appointments and can arrange for transportation for those residents who may not have it readily available.

Residents’ emotional and spiritual needs may be provided for by our Social Worker, Activities Director, Community Leaders, and Faith Leaders.

We always encourage visits from family, friends, and volunteers. Well behaved, vaccinated family pets are also invited for day visits, too.
